Please assess the student performing a back exam.

GENERAL - Observe the student doing the following:

Washes hands
       
Introduces self and exam to patient
               
Appropriate draping of patient

Drape the patient so their back is exposed

               

INSPECTION - The student should comment on the following:

Patient's general appearance

Ex: In pain, well, generally well/unwell etc.

               
Inspects the back

Comments on deformities. Ex: Kyphosis, scoliosis, scars, lesions

               
Inspection of forward flexion

Asks about pain on flexion, comments on abnormalities

               
Inspection of extension

Asks about pain on extension, comments on abnormalities

               

PALPATION - Observe the student performing the following manoeuvers:

Palpate along the spinous processes

Palpating for tenderness or gross deformities

               

MOTOR SCREEN - Observe the student performing the following manoeuvers:

Performs one L4 test on left leg

Can be knee reflex or quadriceps power

               
Performs one L4 test on right leg

Can be knee reflex or quadriceps power

               
Performs one L5 test on left leg

Can be great toe extension, trendelenburg, or ankle dorsiflexion

               
Performs one L5 test on right leg

Can be great toe extension, trendelenburg, or ankle dorsiflexion

               
Performs one S1 test on left leg

Can be great toe flexion, ankle plantar flexion, ankle reflex

               
Performs one S1 test on right leg

Can be great toe flexion, ankle plantar flexion, ankle reflex

               
Performs upper motor neruon test on left leg

Ex: Clonus or describes how to perform plantar response

               

SENSORY SCREEN - Observe the student performing the following manoeuvers:

Saddle sensation (S2

Palpate between the upper buttocks for normal sensation

               

SPECIAL TESTS - Observe the student performing the following manoeuvers:

Straight leg raise

Only positive if leg pain is reproduced

               
Femoral stretch test

Positive test is reproduction of anterior thigh pain. Back pain is not relevant
